The survey window is November 12–25 for all 6th, 8th, and 11th grade students.
Our school is participating in the Student Health Survey (SHS), which is sponsored by the Oregon Health Authority Public Health Division and the Oregon Department of Education.
This online survey will be completed in class and asks about various topics related to student well-being, including:
Health and safety
Mental and behavioral health
School climate and culture
Risk and protective factors
The goal of this survey is to better understand and support the health and well-being of Oregon students.
If you do not want your student to participate, please complete and return the opt-out survey form to Taft Elementary.

Taft Elementary Costume Guidelines
Students are welcome to wear costumes! Please follow these school guidelines to ensure a fun and safe day for everyone:
- Costumes should not disrupt the learning environment.
- No masks or anything that makes it difficult for staff to identify students.
- No weapons, replicas of weapons, weapon-like accessories, or sharp objects.
- Costumes must not demean any ethnic group, race, religion, nationality, disability, gender, or individual.
- Costumes may not depict gangs, terrorist organizations, horror characters, graphic violence, blood, gore, or anything scary in nature.
- Costumes must not display obscene material, prohibited substances, or offensive language.
- Avoid costumes that are too large, have flashing lights, or make sounds that disrupt class.
- Costumes should be tasteful — no underclothing, lingerie, swimsuits, or anything revealing.
- Costumes must not impair vision or movement, and students should still be able to dress out for P.E.
Thank you for helping us keep Halloween fun, safe, and school-appropriate! 🎃
